Advanced Wound Care at Home
Assessment and care for hard-to-heal wounds in the patient’s home.

Wound care at home
Care Plus provides home care for complex and hard-to-heal wounds. The care plan is determined after a professional assessment and may require medical instructions and follow-up.
Conditions that may require assessment
Professional assessment may be relevant for pressure injuries, diabetic foot wounds, surgical wounds, traumatic injuries, burns and wounds associated with venous or arterial insufficiency. This list does not determine suitability for an individual patient.
The care process
The process may include assessment, a documented care plan, dressing changes and follow-up by authorised professionals. Additional tests or consultations are arranged only when clinically indicated by the treating team.
When urgent help is needed
Sudden deterioration, severe bleeding, high fever, confusion or another urgent concern requires immediate contact with the appropriate emergency medical service. The website is not an emergency service.

Frequently asked questions
How do I know whether a wound needs professional care?
A wound that is not healing, is getting worse, is discharging, is causing significant pain or shows a change in appearance warrants a professional assessment. In unusual situations or if it worsens, seek a medical assessment without delay.
Can pressure ulcers be treated at home?
Yes. In suitable cases, pressure ulcers can be assessed, treated and monitored at home.
Can chronic wounds be treated at home?
Many chronic wounds require treatment and follow-up over time, and in suitable cases a significant part of the treatment can be carried out at home.
Does every wound need the same dressing?
No. The type of dressing is chosen according to the type of wound, its condition, the discharge, the surrounding skin and other factors.
Does the dressing need to be changed every day?
Not necessarily. How often the dressing is changed depends on the type of wound, the type of dressing and the treatment instructions.
Can the progress of the wound be monitored?
Yes. In cases of ongoing treatment, the condition of the wound and the changes in it can be professionally monitored over time.
What should I do if signs of infection appear?
When infection or worsening is suspected, an appropriate medical assessment is needed, and it is not advisable to rely on changing the dressing alone.
What is the difference between professional wound care and just changing a dressing?
Professional care starts with an assessment of the wound and the surrounding skin, taking into account the patient’s condition and the medical instructions. The assessment determines the type of dressing, how often the wound is followed up and whether guidance or a further referral is needed. A dressing change is therefore sometimes part of the treatment, but it does not replace a professional assessment when a wound is not healing or is getting worse.
