It depends on the patient’s medical and functional condition, the goals of rehabilitation, the level of assistance required and whether the process can be carried out at home appropriately and safely. After an assessment it can be determined whether the home is suitable for the process, whether adjustments are needed or whether another setting is required.

How to prepare for rehabilitation at home?

Before the assessment it is worth preparing a discharge summary or an up-to-date medical document, a list of medications, previous instructions and information about the assistive devices the patient uses. The team also looks at the living environment and at the activities that matter to the patient in daily life. There is no need to buy equipment before a tailored recommendation has been given.

Care Plus nursing teams provide nursing care and rehabilitation in the patient’s home, subject to a professional assessment and the conditions of the service.

Receiving care at home may reduce the need to travel and allows the patient to remain in a familiar environment.

Who may be referred

The service may be relevant after hospitalisation, surgery, trauma or a stroke, and for people living with complex or chronic conditions. Suitability must be determined by the referring and treating professionals.
