Does Care Plus work with the health funds?

What is Care Plus HomeCare?

HomeCare is the Care Plus division for advanced medical, nursing and rehabilitation services in the patient’s home, with a team of experienced physicians, nurses and therapists and a personal treatment plan in a familiar, comfortable environment.

Which services belong to HomeCare?

HomeCare includes: home hospitalisation, nursing rehabilitation at home, home hospice, symptom management, nurse home visits, wound care, physiotherapy at home and Ferinject iron infusion at home.

Some of the services are provided under contracts with health funds and referring bodies.

How long does rehabilitation at home take?

The length of the process and the frequency of sessions are not the same for every patient. They are set after an assessment and according to the goals of rehabilitation, the patient’s condition, their progress and the service framework.

Acute home hospitalisation

Does home hospitalisation really replace a hospital stay?

In suitable cases, home hospitalisation can serve as an alternative to hospitalisation. The decision depends on the medical condition, the patient’s stability and their suitability for care in the home setting.

Can I receive intravenous treatment at home?

When there is a medical order and the treatment is suitable for delivery at home, intravenous treatments can be given within the appropriate services, in accordance with the treatment plan.

Can pneumonia be treated at home?

In some cases, depending on the severity of the illness, the patient’s condition and a medical assessment, pneumonia can be treated within home hospitalisation. Not every case is suitable for this.

Can dehydration be treated at home?

Which symptoms are involved?

Symptoms that may be addressed, depending on the assessment, include shortness of breath, pain, weakness and fatigue, nausea, swelling (oedema) and a decline in functioning.

Do monitoring and adjustment at home guarantee that hospitalisation will not be needed?

No. Monitoring and adjustment at home do not guarantee that hospitalisation will not be needed. When the patient’s condition requires it, the team will recommend assessment or treatment in the appropriate setting.

Can medications or doses be changed without instruction?

SonoCare — imaging and echocardiography at home

What is SonoCare?

SonoCare is the Care Plus medical imaging division: medical imaging in the patient’s home by a certified team, including echocardiography with professional interpretation by a specialist cardiologist.

Is an echocardiogram at home different from one at an imaging institute?

The principle of the test is the same: ultrasound is used to image the heart. The home service allows the test to be performed at home using a suitable portable ultrasound system.

Is there radiation in an echocardiogram?

No. An echocardiogram is based on ultrasound waves and does not use ionising radiation.

Is special preparation needed before an echocardiogram?

A standard echocardiogram usually does not require complex preparation. If there are special instructions for a particular case, they will be given before the test.

How long does an echocardiogram at home take?

The length of the test can vary depending on the patient and the findings required, so there is no single duration for every test.

Is an echocardiogram at home suitable for housebound patients?

Can I book a cardiologist to come to my home?

Yes. Care Plus provides cardiology services at home, subject to availability and the service area.

Can a cardiologist visit be combined with an echocardiogram?

There are pathways in which a cardiologist’s assessment can be combined with an echocardiogram at home, depending on need and availability.

Is a specialist home visit suitable after discharge from hospital?

What medical information may be used in a remote consultation?

Depending on the subject of the enquiry, the physician may review discharge summaries, laboratory tests, imaging tests, an up-to-date medication list and previous opinions.

Is every medical question suitable for a remote consultation?

No. The physician uses their judgement as to whether a remote response is possible. When the information is insufficient or a physical examination is needed, the patient should be referred for further investigation or an in-person appointment, and a different care setting may be recommended.

How is the medical information transferred for a remote consultation?
